Student Placement - On Q Dance Center is a technique based studio that provides the same professional training whether the student is taking dance as a recreational or casual experience, a competitive experience, or is striving to become a professional. After discussion with the parent, the Artistic Director may place a student in a different class level or style that will achieve the greatest benefit to the child based primarily on the students technical ability, with age of student as a secondary consideration. Students will not be placed into a class strictly at the will of the parent as such non-professional practice may cause harm to the child's ability to learn and their self esteem. Class sizes are limited and number of students per class is decided at the Artistic Directors discretion based on age, ability, and student expectations. Classes may be cancelled if there are 5 or less students enrolled in a class. Optionally, students may be asked by the Artistic Director to combine into a similar type class.
Inclement Weather - On Q Dance Center does not match or follow any specific school district for inclement weather school closings. Although all the schools may be closed, we may be open if the weather has sufficiently improved. Please rely on WTVG Channel 13 news channel and the WTVG Channel 13 website for any class cancelations due to inclement weather.
Observing My Child - Parents have the opportunity to observe their child through a television monitor in the lobby so that the student may get the full uninterrupted benefit of the class. Observance times take place the first two weeks of the month with each class room on a different week. Observance months are October, November, February, March & April. Observance weeks may be reschedule due to holidays or the Artistic Directors discretion.
Students - All students are expected to give the utmost respect to all instructors and fellow students. When performing outside of On Q, students & parents are expected to have good sportsmanship and etiquette. All students are expected to follow the dress code and arrive on time for class. Water is the only food substance allowed in the dance rooms.
Parents:
Communication - Please do not approach the teachers or coaches between classes. Please do not hesitate to leave a message at the desk, in the drop box, or email the Artistic Director if you have any questions or concerns about ANYTHING that involves your child and your child's class, the instructor or coach, or ANY other concern. The Artistic Director will follow up with you ASAP. It is your responsibility to provide us with updated contact information including a valid email address. We communicate via email whenever possible and you will receive your monthly account statement via email. If you do not have an email address it is your responsibility to come into the studio and check the bulletin board or the studio communication binder on the desk for any studio information.
Student Absence - If your child is going to be absent from class due to illness or other reasons, please email us or call the studio and let us know that they will be absent. Students with a fever will not be allowed to take class. Please follow the advice of the CDC where influenza or flu like symptoms are concerned. Behavior - Any malicious or other unwelcomed behavior towards another student, parent, staff, and good name of On Q Dance Center will cause immediately expulsion from the studio at the discretion of the Artistic Director, forfeiting any paid tuition.
Lobby - No parent is permitted to leave a sibling of a student unsupervised in any area of the studio at any time. Food and drink other than water is not allowed in the lobby. If you choose not to respect the rules, then you choose to pay for any service we require to clean or repair damage caused by your spilled foods.
After Class - Parents are expected to pick up their child promptly at the end of their class.
Artistic Director - The Artistic Director decision is final in all instruction matters of the dance studio including but not limited to student placement, music, costumes, choreography, auditions of any type, competition venues, parade and performance venues.
Miscellaneous - We are not responsible for any lost, stolen, or left behind items on or off On Q premises.
- We reserve the right to update this policy as needed.
Payment Terms & Conditions
Registration Fee: I understand there is an annual non-refundable $20.00 Family Registration Fee for Seasonal September through June classes. The $20.00 Registration Fee will immediately be charged to my credit card when I enroll online for classes. If I use the paper Registration Form, the $20.00 Registration Fee will be turned in with the paper Registration Form. There is NOT a Registration Fee charged when enrolling for Summer Classes and Camps.
NOTE: WE NO LONGER ACCEPT CASH FOR ACCOUNT CHARGES.
Payment Options: I understand I have the option to make payments as follows:
1. I can set my account for one of the Automatic Payment (Auto Pay) options in the "e-Payment Schedule" selection box below and pay via my credit card on the 1st day of each of the ten months September through June.
2. By logging into my account through the On Q Customer Portal at onqdancecenter.com, I may pay online with the VISA or MasterCard listed on the “My Account” tab. If I use the paper Registration Form, I understand I must provide a valid email address to use this option. Credit card payments are not accepted at the studio.
3. I may pay by Personal Check or Money Order in person at the studio or by mail. A $20.00 fee will be assessed on all returned checks. I will make checks payable to On Q Dance Center and note the student's name, and account last name if different, in the memo portion of the check. I may mail payments to: On Q Dance Center, 1550 Reynolds Rd., Maumee, OH 43537 in a manner so they are RECEIVED by On Q Dance Center by the 10th of the month so I can avoid the automatic late fee applied to my delinquent account on the 11th of the month.
Auto Pay Selection for Account Charges
1) ALL ACCOUNT CHARGES: By selecting the 1)ALL CHARGES on 1st of Month in the "e-Payment Schedule" selection box, I Authorize On Q Dance Center LLC to charge ALL charges on my account to my credit card provided on the 1st of each month while attending class at On Q Dance Center.
2)TUITION ONLY: By selecting the 2)TUITION ONLY on 1st of Month in the "e-Payment Schedule" selection box, I Authorize On Q Dance Center LLC to charge ALL applicable Seasonal Monthly Tuition (Septemeber - June) to my credit card provided on the 1st of each month while attending class at On Q Dance Center. I understand that I must manually pay for all non-Seasonal Monthly Tuition charges (i.e. Costume Fees, etc) using Payment Options 2 and/or 3 above if I choose this option.
3)NO AUTO PAY: If you do not want to use the Auto Pay feature at this time, choose 3)NO Auto Pay at this Time in the "e-Payment Schedule" selection box. I understand that I must manually pay for all account charges using payment options 2 and/or 3 above if I choose this option.
I understand I may login to my account online and change this setting at any time and that changing this setting does not relinquish my obligation to pay any and all account charges per the On Q Dance Center LLC Policy.
I understand this agreement and credit card authorization is void only: If the card holder or parent of student wishing to discontinue class provides written or email notice of withdrawal to the Artistic Director by the 21st of the previous month. Failure to do so will extend responsibility of tuition. I understand that discontinuing a class does not relinquish my obligation to pay any and all previous charges per the On Q Dance Center LLC Policy.
Seasonal Monthly Tuition Due Date & Late fee: I understand Seasonal Monthly Tuition (SMT) is due the 1st of each of ten months, September through June. I understand there is a grace period from the 1st through the 10th of each month to pay SMT and if I do not make SMT payment in full on or before the 10th of the month, a $5.00 Late Fee will be automatically charged to my account on the 11th of the month. I am not permitted to carry SMT balances or late fees over from month to month. If I do not fully pay my overdue SMT and late fee by the 1st of the next month, my child will not be allowed to take class until my account is brought up to date. I understand Seasonal Monthly Tuition (SMT) is an averaged amount charged each month and it does not fluctuate depending on the number of classes each month. The dollar amount is based off a yearly rate and then split into ten even monthly payments (September through June) for my convenience. I understand ALL Tuition charges & late fees to date must be paid in full for a child to perform in any recital or show.
Company Tuition: I understand Company Tuition (CT) is considered part of the SMT with the exception that no discount applies to the CT rate. I understand that all the above SMT terms and conditions apply to CT.
Summer Tuition: I understand Summer Class andCamp Tuition
Student Absence and Class Cancelation Policy: I understand tuition of any type is not credited, refunded, deducted, prorated, nor are classes rescheduled for classes that are cancelled due to inclement weather, fall on our holiday breaks, or for an absent student due to illness, or any other obligatory conflict. A student is invited to make up a missed class that was missed due to illness or other obligatory conflict within 30 days of the missed class. If applicable, I will discuss this option with the Artistic Director for correct placement in a similar type class. A substitute teacher may be used in lieu of cancelling a class due to instructor illness or other conflicts. If a makeup class is scheduled it will be at the instructor's convenience and it is up to my child to attend that makeup class.
Costumes: I understand my Costume will not be ordered without full payment, this includes sales tax and applicable late payment fee. Once my costume is ordered, there are no refunds or credit of any type given, the costume is mine. If I fail to pay for my costume by the due date I agree to pay an additional $10.00 per costume late fee when I make my payment. If I do not include the late fee with my late payment my costume will not be ordered. If I ask On Q Dance Center LLC to attempt to exchange my costume, I agree to pay all additional shipping and handling charges. On Q Dance Center LLC does not have control over arrival dates, back orders, exchange turnaround times, shipping or any other fees to and from their vendors. Therefore, if I am late with my costume payment my costume may not arrive in time for the purpose it was ordered.
Competition Fees (Applies only to Company Members): I understand that if any Tuition or Competition Costume Fees are not paid in full to date, a student will not be entered into a competition. A student will not be entered into a competition without full payment of that competition's fees. If I pay competition fees by credit card, I understand there is a 4% Convenience Fee charge added to my competition fee payment and I give On Q Dance Center LLC permission to charge my credit card this fee. I have the option to avoid the Convenience Fee by using pay option number 3 above. Any late fee charged to On Q Dance Center LLC will be passed on to me and I agree to pay these charges. Refund of competition fees are bound by the refund policies of the competition firms and any attempt at a refund must go through the Artistic Director.
Account Charge Payment Order: I understand all account payments will be applied to all my outstanding account charges in the following order each month:
1) Tuition & applicable Late Fee
2) Costume & applicable Late Fees
3) Competition Fees
4) All other charges
I authorize the use of photos and video of my child from any and all On Q related events for any and all advertising and promotional purposes at the sole discretion of On Q Dance Center LLC.
As the legal parent or guardian, I understand and acknowledge that there is on and off premise risk of injury, temporary and/or permanent, or even death involved directly and indirectly with the activity for which I am registering my child. I release and hold harmless On Q Dance Center LLC, its owners and operators, and its building owners and operators, from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of On Q Dance Center LLC, its owners and operators or in route to or from any of said premises.
